After a LOOOOONG time, i (with help) figured out how to do it: here is the Inline 4 Piston Combustion Engine Proof of Concept!
Controls:
AG2 for Engine turn off/on.
AG3 for gear transmission (gear ratio of ~11.39:1).
Pitch + for gas.
Pitch - for reverse gas.
Throttle for optional starting engine (~110 rpm) (Not needed).
Sliders 1, 2, 3 and 4 for pistons control (automated by vizzy).
Engine specifications:
Speed: 200 rpm to 1300 rpm (depends almost entirely in your FPS and game time speed).
Quantity of pistons: 4 pistons.
Cylinder layout: Inline.
Piston arrangement: 1-2-3-4.
Obs: It's desirable to set Ultra Physics, no Spike Reduction, Infinite Fuel, 0% Heat Damage and 0% Physics Damage for this build to work well
The main differential of this build, is the use of actual rocket engines to simulate the combustion inside a real piston chamber and mantain a stable rpm without almost any FPS cost and the capability of reverse.

@KashmianAerospace You can try it without cheats, it'll probably break apart due to the rocket engine heat or due to the gear engagement.
