On December 11th, 1998, the first spacecraft of the Mars Surveyor ‘98 mission, the Mars Climate Orbiter, was launched on top of a United Launch Alliance Delta II launch vehicle. The orbiter was to act as an interplanetary weather satellite and a relay for the Mars Polar Lander, another spacecraft for the Mars Surveyor ‘98 mission. Unfortunately, a navigation error that was caused by a translation error caused the orbiter to burn up in the martian atmosphere. Unfortunately, the Mars Polar Lander was also destroyed as well.
